Whatever brings you here, you’ll find the expertise and support that’s right for you.
Short-Term Rehabilitation
After surgery, illness, or injury, the road back to independence looks different for everyone. At McCormick's Creek Rehabilitation and Healthcare Center, your therapy program is built around your specific goals — whether that’s climbing the stairs, getting back to the kitchen, or simply managing your day with confidence. Our therapists bring both the skill to challenge you and the heart to cheer you on.
Therapy is tailored to what matters to you specifically: not a standard program, but one built around your life and what you want to get back to.
We track every milestone and adjust your program as you improve, keeping you and your family informed throughout.
We offer various therapies to address every aspect of your recovery, improving both your physical and your cognitive health.
Skilled nursing means round-the-clock clinical oversight. Licensed clinical staff are on site at all hours, and a supervising physician directs your care plan. Whatever your medical needs, there’s always someone qualified, attentive, and ready.
Directed by your physician, our full team of nursing, therapy, social work and nutrition staff designs, implements and regularly reviews a care plan tailored to your specific needs.
Your health is watched closely at all times: vital signs, wound care, and any change in condition, so anything unusual gets spotted early.
Our clinical team manages every detail of your medication: dosages, interactions, adjustments, and IV medication when required, so you never have to worry about it.
On-site physical, occupational, speech and respiratory therapy to help you rebuild and maintain strength, skills, and independence.
Dementia affects every person differently. The right care is patient, consistent, and deeply attuned to the individual. Our team is trained to provide that skilled, sensitive support in a calm, familiar environment, focusing on comfort, dignity, and quality of life at every stage.
When the focus of care shifts from treatment to comfort, hospice ensures that dignity and peace become the priority — for both residents and their families. We work with a dedicated hospice provider to bring expert end-of-life care directly to our residents, so this chapter can be faced in familiar surroundings, supported by people who know you.
Recovering from a joint replacement, fracture, or orthopedic procedure is demanding — physically and emotionally. Our therapy team works with you with expertise and encouragement, focusing on restoring your mobility and independence.
The days and weeks following surgery are critical. The right monitoring and support during this period makes a real difference to how well and how quickly you recover. Our nursing and therapy teams work together to manage your recovery safely and get you home as soon as you’re ready.
A stroke can affect movement, speech, memory, and daily function in ways that vary enormously from person to person. Our integrated team of nurses and therapists builds a recovery plan around your specific situation, working with skill and patience to help you regain as much independence as possible.
Surgical wounds, pressure injuries, diabetic ulcers, and chronic wounds all require specialist assessment and a structured treatment plan. Our nursing team handles that with the clinical expertise and the attention to detail that proper wound healing demands.
For residents managing pulmonary conditions, our respiratory team works closely with your physician to build a plan that keeps breathing as comfortable and easy as possible, every day.
Recovering after a cardiac event or managing a long-term heart condition takes careful monitoring and steady, unhurried support. Our nursing team works with your cardiologist to keep medication, activity and diet on track, watching for the small changes that matter before they become setbacks.
Pain that is not properly controlled slows everything else down, including therapy. Our team assesses it honestly and often, combines medication with hands-on and therapeutic approaches, and adjusts the plan as your recovery changes rather than leaving you to manage on a schedule set weeks ago.
